Referring Physician

We commit to performing the examination requested within 48 hours following the receipt of the authorization from the CSST agent responsible for the worker’s file. We will fax you the results within 48 hours following the examination you prescribed.

Worker

To schedule an appointment, you have to provide us the following information:

• Referral from your physician
• Your CSST file number
• The name of your CSST agent
• Your CSST agent’s fax number
